Sarah Revis

Administrator

Sarah serves as the Administrator at NorthChase Nursing & Rehabilitation Center, where she leads a dedicated team of caring individuals. Her journey with senior care began with a childhood spent at local nursing homes and the YMCA, where she found joy in brightening the lives of residents. Influenced by her grandparents, Sarah felt a deep calling to improve the lives of the elderly. She pursued a degree in Public Health at East Carolina University (ECU), during which time she interned at a nursing home, eventually becoming an Activities Director. Continuing her education, Sarah earned a Master’s in Health Administration from ECU. She later became a licensed Nursing Home Administrator and gained valuable experience in interim roles at sister facilities. Sarah’s mission is to lead with integrity and compassion while delivering top-quality care to residents, families, and staff. She is deeply connected to the local Wilmington community and is dedicated to making a positive impact on the lives of its residents.

Alexandria Baker

Assistant Administrator

Alexandria proudly serves as the Assistant Administrator at NorthChase Nursing & Rehabilitation Center. Her journey into healthcare and senior care began during her high school years when she enrolled in a Certified Nursing Assistant Program and spent her final two years assisting at a local nursing home. This early experience ignited her passion for helping others and set the foundation for her future career in healthcare. After high school, Alexandria pursued a degree in Public Health with a pre-medical concentration at ECU. During her Master’s program, she was invited to participate in the Administrator in Training program with NorthChase’s management company, a challenge she wholeheartedly embraced. Following her graduation in 2017, her career took off as she assumed her first building. Over the years, she has had the privilege of working in various locations across North Carolina and Virginia, accumulating a wealth of experience and insights from each unique facility. Her mission is to lead by example, cultivate a culture of appreciation, and leverage her diverse experience to deliver the highest quality of care. She is deeply committed to making a positive impact on the lives of residents, families, and staff, working collectively to ensure the well-being and happiness of all.

Amy Lieseke

Director of Nursing

Amy began her career in healthcare journey as an LPN in long-term care, learning the clinical operations from the ground up. Fueled by her passion for nursing, she set her sights on becoming an RN and further advancing her career. Her dedication quickly propelled her into a management position at a local psychiatric facility, where she achieved the honor of being voted Employee of the Year in her first year as an RN. Amy’s experience in psychiatric care provided valuable insights that have enriched her overall career. She later transitioned back to long-term care, this time in a leadership capacity as a unit manager, gradually evolving into the role of Director of Nursing where she is deeply committed to fostering a positive culture that supports teamwork and collaboration. Her genuine enthusiasm for getting to know each resident who enters our doors reflects her belief in the significance of every individual’s life, regardless of the duration of their stay.

Admissions Director

Holly began her healthcare career as a Certified Nursing Assistant (CNA) in private care before joining a long-term care facility in Burgaw, North Carolina, as a housekeeper. Recognizing her potential, an Administrator provided her with an opportunity in the admissions department. Holly’s journey continued as a Marketing Director for an assisted living company and ultimately as an Admissions Director for multiple facilities. Having experienced the loss of her grandmother at a young age, Holly now sees herself surrounded by a community of “grandparents” at NorthChase and her mission is to extend a warm and loving welcome to every resident, ensuring they feel cherished at our center.
